Registration and Orientation Week

The registration for the NURSING program is open now and takes place at the CAMPUS, 94A Okrzei Street
Room number 14, office hours 9-15

Required documents:

• completed and signed application to the Rector according to the model of the University – APPLICATION
• a copy of the secondary school-leaving certificate (originals available for inspection when submitting documents),
• copy of high school graduation certificate (originals available for inspection when submitting documents),
• 2 identical, current photographs, in accordance with the requirements applicable to the issuance of identity cards, signed on the back with the name and surname of the candidate for studies.
• digital photo (JPG format, size should not exceed 50 kB (400×500 pixels) on a CD or DVD
• identity card, and in the case of candidates for studies who are foreigners, another document confirming the identity of the candidate for studies, available for inspection when submitting documents.

  * documents submitted in a foreign language must be translated into Polish by a sworn translator

The student ID will be ready after submission of the above within 2 weeks, the cost is 30 PLN
